Client Agreement & Informed Consent

T1D & Celiac Peer Coaching, Individual, Family, and Travel Prep Sessions

1. Nature of Services

Cole's Coaching provides peer coaching based on lived personal experience with type 1 diabetes and celiac disease. Services may include individual coaching, parent/family coaching, and travel preparation coaching, delivered virtually or in person.

Cole's Coaching is not a licensed medical provider, registered dietitian, certified diabetes care and education specialist (CDCES), therapist, or counselor. Nothing provided in a coaching session constitutes medical advice, diagnosis, or treatment.

2. Not a Substitute for Medical Care

Client agrees to continue working with their own physician, endocrinologist, dietitian, or other licensed medical provider(s) for all medical decisions, including but not limited to medication, insulin dosing, dietary restrictions, and treatment plans. Client will not make changes to medication, dosing, or medical treatment based solely on information discussed in coaching sessions.

This service is not equipped to respond to medical emergencies. If Client or Client's family member is experiencing a medical emergency, Client agrees to call 911 (or the applicable local emergency number) or go to the nearest emergency room immediately, rather than contacting Cole's Coaching.

3. Assumption of Risk

Client understands that coaching sessions may involve discussion of health conditions, travel planning, and related topics, and that Client is voluntarily choosing to participate. Client assumes full responsibility for any decisions made or actions taken based on discussions during coaching sessions.

4. Release of Liability

To the fullest extent permitted by law, Client releases and holds harmless Cole's Coaching, its owner, and any associated parties from any and all claims, damages, or liability arising from participation in coaching services, except in cases of gross negligence or willful misconduct.

5. Confidentiality

Cole's Coaching will keep the content of coaching sessions private and will not share identifying details with third parties, except: (a) with Client's written permission, (b) where disclosure is required by law, or (c) where Cole's Coaching believes there is an imminent risk of serious harm to Client or another person.

6. Minors

For clients under the age of 18, a parent or legal guardian must review and sign this agreement on the minor's behalf. Parent/guardian involvement in scheduling and, where appropriate, session content is expected for minor clients.

7. Cancellations & Scheduling

Sessions may be cancelled or rescheduled at no charge with at least 48 hours' notice before the scheduled session time. Cancellations made less than 48 hours before the scheduled session, including no-shows, will result in forfeiture of payment for that session.

8. Payment

Payment is due in full before the session begins via Venmo. Venmo handle will be provided at booking. No session will be held until payment has been received.

9. Governing Law

This agreement is governed by the laws of the State of California.

By completing and submitting this form, you (or your parent/legal guardian, if you are a minor) confirm that you have read, understood, and agree to the terms above.
